T&A Safety Services offer professional safety advisory and consultancy services, and conduct independent incident investigations, building on 27 years’ health and safety experience in the rail industry to drive effective safety improvements, performance and learning.

Peer review and feedback on your investigation reports

We can offer you practical support and advice with an independent peer review of your internal investigations, prior to the final investigation report submission to your client. This facilitates an impartial review that will ensure the investigation:

has met all the necessary requirements
has got to root cause
will be effective in enabling learning
has effective, evidence based corrective actions
enables safety improvements and will reduce the likelihood of the accident or incident reoccurring.

Mentoring and coaching for newly trained investigators or to enhance existing investigator skill sets

We can offer advice and guidance to your team of investigators, that will help improve their approach to incident investigation and their understanding in undertaking effective investigations that get to root cause.

Michelle Stephens MSc CMIOSH

Michelle Stephens is a highly skilled safety investigator trained to MSc level at Cranfield University and specialises in conducting incident investigations and delivering bespoke IOSH approved Incident Investigation training courses. As an experienced Chartered Safety and Health Practitioner, Michelle has been operating in the rail industry at senior management level in both corporate and operational health and safety, for more than 27 years.

Michelle also offers professional safety advisory and consultancy services including developing, writing and auditing safety, quality, and environmental management systems, to recognised international standards, and delivering a wide range of safety services and support.

Michelle studied for and achieved her Degree of Master of Science in Safety and Accident Investigation (Rail Transport) at Cranfield University between 2017 and 2021. She has also achieved the Award in Education and Training in 2018.
